Welcome to Cedar Creek Stingrays Online Registration

**Important** This season will be like no other due to the covid situation. This a fluid situation and always changing.

This Registration is for those that are members of CCSRC or who want to be First Time new members this year. If you don't want to be a new member,  or are Not a member, please go to the Non Members registration link. 

You can join our summer swim team. You do not need to be a member of the Cedar Creek Swim and Racquet Club to participate on the team, but families who join as annual members do receive a discount on swim team registration fees.

This season will be differently than any season we have had before. This year we will have 4 virtual meets. All meets will be at Cedar Creek and we will run the meet following CDC guidelines. Once it is over, it will be uploaded and merged with our opponents results. There will be a county meet at Georgia Tech.

If you forgot your password, go to login page and reset your password and then login to register. If a parent is logged into their Swimtopia account and visits the registration page, we will automatically pre-load data from the prior year's registration (if any). I live within the Cedar Creek or Cedar Creek Crossing subdivision and my 2021 Cedar Creek Swim & Racquet Club (CCSRC) membership dues are current or I do not live in Cedar Creek or Cedar Creek Crossing subdivision but my 2021 CCSRC guest membership dues are current or will be. If later it is found I am in fact not a member of CCSRC, applicable Non Member charges can be applied later.

REQUIRED VOLUNTEERISM

I acknowledge that the Cedar Creek Swim and Racquet Club, the CCSRC Stingrays Swim Team and the Gwinnett County Swim League are volunteer driven organizations. It is the obligation of every family to provide one adult at all home meets and at some away meets for those dual meets in which the registered swimmer(s) participates. I further understand that it is my responsibility to find a suitable replacement should I not be able to work my assigned shift. If I fail to serve my role or find a replacement for my assignment, my swimmer(s) will not be allowed to participate in the next scheduled meet.

REGISTRATION REQUIRED PRIOR TO PARTICIPATION

I acknowledge that because of GCSL rules, swimmers are not considered registered and are not allowed to participate in any swim team or league related activities until all paperwork and payment is received.  New swimmers *ONLY* to the Gwinnett County Swim League must show a certified original and provide a copy of their birth certificate to a council person. This is not required if the swimmer was on a GCSL team in 2021.

2021 Athlete/Parent Concussion Awareness Form

DANGERS OF CONCUSSION

Concussions at all levels of sports have received a great deal of attention and a state law has been passed to address this issue. Adolescent athletes are particularly vulnerable to the effects of concussion. Once considered little more than a minor “ding” to the head, it is now understood that a concussion has the potential to result in death, or changes in brain function (either short-term or long-term). A concussion is a brain injury that results in a temporary disruption of normal brain function. A concussion occurs when the brain is violently rocked back and forth or twisted inside the skull as a result of a blow to the head or body. Continued participation in any sport following a concussion can lead to worsening concussion symptoms, as well as increased risk for further injury to the brain, and even death.

Athlete and parental education in this area is crucial—that is the reason for this document. Refer to it regularly. This form must be signed by a parent or guardian of each swimmer who wishes to participate in GCSL activities. 

COMMON SIGNS AND SYMPTOMS OF CONCUSSION

● Headache, dizziness, poor balance, moves clumsily, reduced energy level/tiredness

● Nausea or vomiting

● Blurred vision, sensitivity to light and sounds

● Fogginess of memory, difficulty concentrating, slowed thought processes, confused about surroundings or assignments

● Unexplained changes in behavior and personality

● Loss of consciousness (NOTE: This does not occur in all concussion episodes.)

Media Consent Form and Release

 I hereby grant Gwinnett County Swim League and their agents, representatives, contractors and/or members the absolute right and permission to use photographic portraits, pictures, digital images or videotapes of my child, or in which my child may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in advertisement or promotional material such as Gwinnett County Swim League publications or the Gwinnett County Swim League website, without payment or any other consideration.

I hereby release, discharge and agree to indemnify and hold harmless Gwinnett County Swim League and their agents from all claims, demands, and causes of action that I or My Child have or may have by reason of this authorization or use of My Child’s photographic portraits, pictures, digital images or videotapes, including any liability by virtue of any blurring, distortion, alteration, optical illusion or use in composite form, whether, intention or otherwise that may occur or be produced in the taking of said images or videotapes, or in processing tending towards the completions on the finished product, including publication on the internet, or any other advertisements or materials.

I represent that I am at least eighteen (18) years of age and am fully competent to sign this Release.

This is a release of legal rights. Read it carefully and be certain you understand before signing.

2021 League Waiver

LIABILITY RELEASE AND INDEMNIFICATION FORM

I, the undersigned participant and parent, request voluntary participation for minor to participate in all events, which are hereinafter referred to as the “activities.” sponsored by Gwinnett County Swim League (GCSL) and its individual swim teams. This agreement is valid while the participant is a registered participant with Gwinnett County Swim League.

I consent to my/minor’s participation in the activities and acknowledge that the minor and I fully understand my/minor’s participation may involve risk of serious injury or death, including losses which may result not only from my/minor’s own actions, inactions or negligence, but also from the actions, inactions, or negligence of others, the condition of the facilities, equipment, or areas where the event or activity is being conducted, and/or the rules of play of this type of event or activity. I understand that if I have any risk concerns, I should discuss the risks associated with my participation with the activity coordinators and event staff, before I sign this document and before any activities begins.

Release – Minor’s Rights:

In consideration of allowing Minor Participant to participate in the activities, I hereby release and hold harmless Gwinnett County Swim League, its individual swim teams, and their members of its board of directors, officers, employees, volunteers, other participants, and agents (collectively, the “Released Parties”), of and from, and do discharge and waive, any and all claims, demands, losses, damages, and liabilities that Minor Participant may have or sustain with respect to any and all damage and/or injury, of any type, arising out of his or her participating in the activities. I also agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

Release – Parents’/Guardians’ Rights: 

In consideration of allowing Minor Participant to participate in this Gwinnett County Swim League season, I hereby release and hold harmless the Released Parties, of and from, and do discharge and waive, any and all claims, demands, losses, damages, and liabilities that I may have or sustain with respect to any and all damage and/or injury, of any type, arising from Minor Participant’s participation in the activities. I also agree that if any portion of this agreement is held to be invalid the balance, notwithstanding, shall continue in full force and effect.

I certify that my/minor is in good health and have no physical condition that would prevent participation in this activity. Furthermore, I agree to use my/minor’s personal medical insurance as a primary medical coverage payment if accident or injury occurs. I consent to emergency medical treatment in the event such care is required.

Indemnification by Parent/Guardian:

The undersigned parent/guardian further agrees to indemnify, save and hold harmless the Released Parties from any and all claims, demands, losses, damages and liabilities for indemnities, contribution or otherwise with respect to any damage and/or injury, of any type, arising from Minor Participant’s participation in the activities. The undersigned also agrees that this Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement extends to all acts of negligence by the Releasee and is intended to be as broad and inclusive as is permitted by the laws of the State in which the Event(s) is/are conducted and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

2021 Covid Waiver

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. The state of medical knowledge is evolving, but the virus is believed to spread from person-to-person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air. People reportedly can be infected and show no symptoms and therefore spread the disease. The exact methods of spread and contraction are unknown, and there is no known treatment, cure, or vaccine for COVID-19. Evidence has shown that COVID-19 can cause serious and potentially life-threatening illness and even death.

Gwinnett County Swim League (GCSL), cannot prevent you (or your child(ren)) from becoming exposed to, contracting, or spreading COVID-19 while participating in Gwinnett County Swim League events. It is not possible to prevent against the presence of the disease. Therefore, if you choose to participate in a GCSL event, you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.

ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk of contracting COVID-19 for myself and/or my children in order to participate in a GCSL event. Participating in the event is of such value to me and/or to my children that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to participate.

Cedar Creek Covid Waiver

Cedar Creek Swim & Racquet Club SWIM TEAM Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19 The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments as well as federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Cedar Creek Swim & Racquet Club located at 853 Angevine Ct SW, Lilburn, GA 30047 has put in place preventative measures to reduce the spread of COVID-19; however, Cedar Creek Swim & Racquet Club cannot guarantee that you will not become infected with COVID-19. Further, entering the pool area could increase your risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by entering the pool area of Cedar Creek Swim & Racquet Club and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at Cedar Creek Pool may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Cedar Creek Swim & Racquet Club Board, Swim Team Council, Swim Team Coaches, Swim Atlanta, and Club Members and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at Cedar Creek Swim & Racquet Club. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless Cedar Creek Swim & Racquet Club, its Board, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Cedar Creek Swim & Racquet Club, Board, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation at Cedar Creek Swim & Racquet Club Pool. I agree to the above Assumption of Risk & Waiver of Liability Relating to Coronavirus/COVID-19.
